What information can be found in my health record ?

health record is created any time you see a health professional such as a doctor, nurse, dentist, chiropractor, or psychiatrist. You could find the following in your health record:

  • Your medical history and your family’s medical history
  • Labs and x-rays
  • Medications prescribed
  • Alcohol use and sexual activity
  • Details about your lifestyle (smoking, exercise, recreational drug use, high-risk sports, stress levels)
  • Doctor/nurse notes
  • Results of operations and proceduresGenetic testing
  • Research participation
  • Driver’s License
  • Social Security Number
  • Financial information such as credit cards and payment info

Who can provide home care?

Home care services are normally provided by Certified Home Care Agencies, Licensed Agencies or agencies that can provide specialized services or products.

Brainz is licensed as a Class A professional Home Care Agency and a PCA/PCPO provider agency. Brainz provides both skilled Home Care Services that require the services of a nurse or a therapist and non-medical services ranging from assistance with activities of daily living, Home management services to companionship services.

What is home care?

Home Care is the service of nurses, therapists, social workers and support services, under the guidance of your physician, in the comfort of your own home where you can be surrounded by the love and support of your family and friends.

Home Care allows you to live independently in your own home, or in the home of a family member, and have access to outside supportive care and assistance when you need it.

People who are recovering from an illness, accident or hospitalization usually use home care services. Services are provided when you can remain safely at home, but still need the ongoing care of a registered nurse or therapist.

With the decreasing Length of stay in hospitals, more patients are turning to Home Care for assistance.

How quickly can I begin my services with you?

It is possible that we can begin services on the same day or next day.  We will come out to your home to complete a free assessment and we will then try to find the right resource for you.  It may take longer for services to begin for Medical Assistance cases, unless a case manager authorizes hours to begin on a certain date.  We can help you navigate through this process as quickly as needed.

How is home care paid for?

Home Care is paid commonly by Private pay (Self pay), insurances and various State and Government Medical Assistance (MA) programs. If you need help, we will take care of you and talk payment later. Please feel free to call and check if we accept your insurance. If we do not, we may be able to tell you who can.

How do I obtain home care services?

Our staff will ask you your basic information re: name, address, phone number and doctor’s name so we can obtain orders. Our staff will also ask you what your concerns are for the patient so we can adequately provide you with the services you will need.

Our nurses will then call your doctor to obtain orders and arrange for a visiting nurse to visit at the patient’s home usually within 24- 48 hours.
